This Program provides the following services:
- Housing Transition Navigation services.
- Housing Tenancy and Sustaining Services.
Who qualifies for the
this
Program?
- Any adult experiencing homelessness and meets the HUD definition of homeless.
- At risk of becoming HUD homeless.
- Exiting an institution such as jail, hospital or SNF after more than 90 days and was HUD homeless prior to entering an institution and would become homeless immediately upon release.
- Meets the HUD definition of chronic homeless.
Other eligibility criteria:
- Eligible or enrolled in ECM homeless population of focus.
- Has two or more chronic conditions.
- High utilizer, defined as: 7 or more ER visits in prior 12 month period.
- 2 or more inpatients visits and/or short term skilled nursing facility in prior 12 month period.

- Assistance obtaining stable housing.
- Assistance with searching for housing
- Assistance with filling out housing applications.
- Developing an individual housing support plan.
- Transitional housing placement.
- Shelter placement.
- Transportation to view properties.
- Financial literacy support.
- Assistance with maintaining stable tenancy, once housing is secured.
- Assistance accessing community resources, such as GR, TAP cards etc.
